I (Parent or Guardian above), hereby release MC2, Inc. from any liability and grant permission for the utilization of any photographic images of my child (name above), by MC2, Inc. In accordance with the marketing and advertising of MC2, Inc. statewide "after-school," Saturday, summer, and special events, focusing on education, sports, field trips, and orkshops held in conjunction with all program sites throughout Connecticut. Such utilization may include, but is not limited to, application forms, brochures, event posters, television, or other print media.
I further agreed to defend, indemnify, and hold harmless MC2, Inc., from any claims or causes of action whatsoever that may result from utilization of any photographic images of my child.
